Sports Physicals

Athletes do not need to be an established patient to schedule. The visit often only takes 15 minutes.
Oregon schools require students who participate in extracurricular sports in grades 7 through 12 to have a physical examination once every two years. We have the appropriate forms in our office. The first page of the form is to be completed by the parent/guardian. The following is a link to a pdf of this form: link.
